Periodontal Plastic Surgery for the Implant Patient

Synopsis

This presentation will review the anatomic basis and biologic principles for application of periodontal plastic surgery in implant therapy. Principles of oral soft tissue grafting and details of current techniques will be presented as they apply around natural dentition and dental implants. A philosophy of care for esthetic implant therapy will be introduced along with a review of special treatment planning considerations that commonly jeopardize esthetic outcomes. Applications of plastic surgery principles and grafting biotechnology in esthetic implant therapy will be demonstrated and six distinct classifications of alveolar ridge defects in esthetic areas will be introduced and correlated with treatment algorithms for successful reconstruction. Strategies to overcome pre-existing anatomic limitations will also be presented.

Learning Objectives

At the conclusion of this presentation, participants should be able to:

  1. Discuss the anatomic basis and principles of oral soft tissue grafting as well as the clinical relevance of biologic width.
  2. Master the indications, surgical technique, and limitations of the following oral soft tissue grafting techniques: gingival grafts, subepithelial connective tissue grafts, onlay-inlay grafts, use of dermal matrix grafts, and pedicle flaps such as the VIP-CT flap.
  3. Discuss: esthetic risk assessment, applications of plastic surgery principles in implant surgery, the role of grafting biotechnology, classification of alveolar ridge defects in esthetic areas, details of esthetic implant site development algorithms, the use of cosmetic periodontal surgery, and the importance of site preservation to achieve predictable outcomes in esthetic implant therapy.
