Encounter Forms

In response to member requests for assistance in implementing more comprehensive record-keeping and documentation systems in their offices, AAOMS has developed encounter form templates customizable to meet your individual practice needs. Since no two practices are alike, you are free to adapt or modify the suggested items on each form for use in your individual practice. As such, these encounter forms, also known as "super bills", are void of any procedural or diagnostic codes. It is the responsibility of each practice to update these forms with the codes annually. Many practice management software programs will allow you to assign internal codes to your procedures and the system will then properly assign the procedural code. So, feel free to download the forms and start using them today!
