Application Requirements: To be considered, oral and maxillofacial surgeons must:
- Submit the online application.
- Be deemed eligible as specified in the “Eligibility” section of the Rules.
- Use the grants as specified in “Use of Grant” section of the Rules.
Eligibility: Oral and maxillofacial surgeons who meet the following requirements may be eligible for the AAOMS—Henry Schein Global Outreach Program. To be eligible OMSs must:
- Be a fellow or member in good standing of AAOMS.
- Hold an active DEA Number.
- Provide a verifiable agreement with a governmental organization, healthcare program overseen by a charitable or not-for-profit organization, or Mercy Ships International to provide voluntary OMS services.
- Complete and submit the Program’s Online Application Form.
- Optional: A letter of interest may be submitted to detail any additional or expanded information that is pertinent to the project.
Use of Grants:
- $2,500 cash grants must be used to support the Recipient's attendance and/or participation in the Program.
- Recipients may select products from the Henry Schein catalog valued at up to $2,000. Selected products must be used on the mission specified in the Recipient’s application.

Award Notification: Selected Recipients will be notified after September 20
Selection: Selection of Recipients to receive a grant from AAOMS—Henry Schein Cares Foundation Global Outreach Program is the responsibility of the AAOMS Board of Trustees. The AAOMS retains the right at its sole discretion to determine eligibility and reserves the right to disqualify any applicant for any reason. All decisions by the AAOMS Board of Trustees are final, binding and conclusive.
Removal from the Program: If a Recipient is deemed to be ineligible for any reason, it is grounds for Removal from the Program without notice. "Removal from the Program" means all monies and the cost of product must be refunded to AAOMS and the Henry Schein Cares Foundation.
Complete and Submit a Project Report: After Recipients have completed the service project for which the grant was provided, they shall submit a brief report on the service project, the patients treated, the procedures performed, the outcome of the project, and how the Program contributed to the success of the service project.
Promotion: Recipients agree to permit AAOMS and the Henry Schein Cares Foundation to use the Recipient’s name and likeness in any and all media, press materials, websites, photos, videos and any collateral materials (except where prohibited by law).
Limitation of Liability: AAOMS and the Henry Schein Cares Foundation, in their sole discretion, reserve the right to suspend, cancel or modify the Program without notice or liability if the integrity of the Program is compromised in any manner as determined by the AAOMS and the Henry Schein Cares Foundation, or if the Program is not capable of being conducted as intended by AAOMS and the Henry Schein Cares Foundation.
Release and Indemnity Release: You release AAOMS and the Henry Schein Cares Foundation, and their parent, affiliates, subsidiaries, officers, directors, agents, employees, service providers, representatives and all others associated with the development and execution of the Program, from any and all liability with respect to, or in any way arising from, the Program, including, but not limited to, any suspension, modification or cancellation of the Program, Content received or posted, and/or acceptance or use of the grant, including liability for personal injury, death, damages or loss.
Indemnity: By participating in the Program, Recipient agrees to indemnify, defend and hold harmless AAOMS and the Henry Schein Cares Foundation, and each of their agents, service providers and their respective parents, and their subsidiaries, affiliates, successors, licensees, and assigns, and each of their directors, officers, agents, equity holders and employees, from and against any and all claims, losses, costs, damages, liabilities and costs and expenses (including reasonable attorneys’ fees and costs) that arise out of your participation in the Program or any breach of the Program Rules.
Award Recipients must complete an IRS form W-9 before award funds can be paid. The award is considered taxable income. The Award Recipient will receive a 1099 tax form from AAOMS for the year the award is received per section 6041 of the U.S. Internal Revenue Code.
Governing Law and Disputes Except Where Prohibited: Recipient agrees that all issues and questions concerning these Program Rules and the Program shall be governed by and construed in accordance with the laws of the State of Illinois without giving effect to any choice of law or conflict of law rules. Recipient and AAOMS and the Henry Schein Cares Foundation each waive all rights to trial in any action or proceeding instituted in connection with the Program (including any activity related thereto). Any controversy or claim arising out of or relating to the Program shall be settled by binding arbitration in accordance with the commercial arbitration rules of the American Arbitration Association. Any such controversy or claim shall be arbitrated on an individual basis, and shall not be consolidated in any arbitration with any claim or controversy of any other party.
